COURSE REGISTRATION FORM

(For MULTIPLE PARTICIPANTS)

If you are just registering only one participant, please click on 'Individual Participant' to open a different registration form.

NOTES:

- All participants must be for the same Course Type, Date, Location and Payment Method. If needed, a second multiple registration can be done for a different Course Type, Date, etc.

- The email confirming the course registration will ONLY be sent to the email provided in the 'Confirmation Email Address' field. Whoever is submitting the form should ensure that all participants they are registering know the time and location of the course and have ensured that the participants meet any prerequisites for the course (ie not expired if doing a recertification).

PLEASE READ BEFORE CONTINUING:

1) If the e-mail address you enter on the on-line registration form is incorrect you will NOT receive your confirmation.

2) Please make sure you are selecting the right Course Type, Date & Location. LMac Community CPR is NOT responsible if you select the wrong option nor liable for any costs those errors may incurr.

4) In keeping with most current workplace policies, the LMac training room is a scent free environment so please refrain from using perfumes, colognes, creams or any thing else that could aggravate a scent sensitivity.
